Job Requirements
- High school diploma or GED
- Current BLS and ACLS certifications
- Valid paramedic licensure in state of employment
- Ability to obtain community paramedicine certification or complete NEMSA Essentials Course within two years
- Effective verbal, written, and interpersonal communication skills
- Demonstrated coaching and leadership ability
- Teaching and educational facilitation skills
- Ability to adhere to educational quality standards
- Familiarity with basic computer applications
- Team-oriented and able to work effectively with others
- Valid driver's license with acceptable driving record

Job Duties
- Provide high-acuity care in patient's home setting for conditions such as infections, heart failure, pneumonia, and post-surgical recovery
- Conduct thorough physical exams and assess home environment for safety and recovery factors
- Collaborate and maintain communication with virtual care team including nurses and physicians
- Respond to crisis situations and rapid response alerts with necessary interventions
- Educate patients and caregivers on disease management, medication, and treatment expectations
- Document all assessments, interventions, communications, and outcomes in electronic health records
- Collaborate with physicians, case managers, social services, and community partners for care continuity
- Participate in program development, research initiatives, mentor new staff, and support community outreach as assigned
