Job Requirements
- High school diploma or GED
- current BLS and ACLS certifications
- required paramedic licensure for state employed
- ability to obtain Community Paramedicine certification or complete NEMSA Essentials Course or obtain CP-C licensure within two years of hire
- effective communication skills
- demonstrated coaching and leadership
- teaching skills
- adherence to educational quality standards
- basic computer application familiarity
- team-oriented
- valid driver’s license with acceptable record
Job Qualifications
- High school diploma or GED
- current BLS and ACLS certifications
- PHTLS and PALS certifications preferred
- required licensure to perform as a paramedic within the state of employment
- effective verbal, written, and interpersonal communication skills
- demonstrated coaching and leadership abilities
- teaching and educational facilitation skills
- familiarity with basic computer applications
- team-oriented and able to work effectively with others
- valid driver’s license and acceptable driving record
Job Duties
- Provide high-acuity care in the patient’s home for conditions such as infections, heart failure, pneumonia, and post-surgical recovery
- conduct thorough physical exams and assess home environment for safety and factors impacting recovery
- maintain real-time communication with virtual care team to report findings, collaborate on care decisions, and receive treatment orders
- respond to crisis situations and rapid response alerts in the home
- deliver personalized patient and caregiver education on disease management, medication, and treatment expectations
- document assessments, interventions, communications, and outcomes in electronic health records
- collaborate with interdisciplinary professionals to ensure continuity of care
- participate in research initiatives and program development, mentor new staff, and represent the program in community outreach activities
