Job Requirements

  • High school diploma or GED
  • Current BLS and ACLS certifications
  • Required paramedic licensure within state of employment
  • Ability to obtain Community Paramedicine certification or complete NEMSA Essentials Course within two years
  • Effective communication skills
  • Ability to provide coaching and leadership
  • Teaching and educational facilitation skills
  • Familiarity with basic computer functions
  • Team-oriented and collaborative
  • Valid driver's license and acceptable driving record

Job Qualifications

  • Required paramedic licensure in state of employment

Job Duties

  • Provide high-acuity care in the patient's home for conditions such as infections, heart failure, pneumonia, and post-surgical recovery
  • Conduct thorough physical exams and assess home environment for safety and recovery impact
  • Serve as an on-site extension of the virtual care team, coordinating with remote nurses and physicians
  • Respond to crisis situations and rapid response alerts with necessary interventions
  • Provide personalized education to patients and caregivers on disease management and medication regimens
  • Document assessments, interventions, and communications in electronic health records
  • Collaborate with healthcare providers and community partners to ensure continuity of care
  • Participate in research, program development, mentoring, and community outreach activities
