Job Requirements

  • associates or bachelor's degree required

Job Duties

  • develop and execute outreach strategies to increase awareness of the community's services
  • build and maintain relationships with hospitals, physicians, discharge planners, case managers, and other referral sources
  • represent the community at community events, health fairs, senior expos, and networking meetings
  • conduct tours of the healthcare center and assisted living for prospective residents, families, and referral partners
  • collaborate with the admissions and nursing teams to ensure smooth transitions for new residents
  • track outreach activities, referral sources, and conversion rates using CRM tools
  • provide education about assisted living and skilled nursing services to community partners
