Job Requirements
- Located in Cook County or surrounding area
- Willingness to travel up to 70 percent in a 90-mile radius
- Possess reliable transportation
- 1-3 years relevant experience such as community outreach or home visits
- Experience with Medicaid populations in urban or rural settings
- Ability to work flexible hours including occasional weekends
- Strong interpersonal and communication skills
- Capacity to adapt to dynamic and unknown working conditions
- Valid certifications as required locally such as first aid, CPR, TB screen
Job Qualifications
- High school diploma or G.E.D
- 1-3 years relevant experience in community outreach or home visits to members
- Experience serving Medicaid populations in urban or rural environments
- Knowledge of local community resources and healthcare programs
- Ability to communicate effectively across diverse groups
- Preferred: Community Health Worker Certification or equivalent training
- Preferred: Mental Health First Aid, Trauma Informed Care, HIPAA, C.L.A.S. Standards, Motivational Interviewing
- Computer proficiency including Excel and Electronic Health Records
Job Duties
- Serve as a bridge between community members and healthcare providers
- Conduct outreach and education to support member health outcomes
- Communicate medical information effectively to healthcare professionals and care coordinators
- Navigate and connect patients with formal and informal community resources
- Travel up to 70 percent within a 90-mile radius of Cook County to visit patient and office locations
- Collaborate within a multidisciplinary team and manage multiple priorities
- Adapt interventions based on members' cultural, ethnic, or religious preferences
