Job Requirements

  • Maintain certificates required for the position including but not limited to First Aid and CPR
  • valid Texas driver's license with a clear driving record and reliable transportation
  • pass a pre-employment drug screen and random drug screens
  • provide proof of work eligibility status upon request
  • pass a pre-employment and biennial criminal background and motor vehicle check (fingerprints needed if applicant has lived out of state within the past 5 years)

Job Qualifications

  • High school diploma or equivalent
  • one or more years of college-level coursework in a related field preferred
  • two years of job experience with a high school diploma or equivalent
  • ability to communicate effectively in writing and verbally in English
  • ability to work independently and as part of a multidisciplinary team
  • computer literacy required to meet job responsibilities
  • cultural competence to respond sensitively to diverse populations
  • ability to work in a fast-paced environment

Job Duties

  • Outreach and recruitment of participants for the HEART program
  • establish and maintain a trusting and professional working relationship with families by providing regular contact
  • record all group and individualized services and activities in a complete, concise, and timely manner utilizing program forms into Salesforce (BCFS) databases
  • maintain a list of applications' status and update the Salesforce database
  • provide safe, secure, and supervised transportation for the service population assigned to this position
  • provide referrals to participants based on needs identified while conducting outreach efforts
  • assist with coordinating and promoting HEART sponsored events and meetings, including classes and community events
  • build collaborative relationships with the community, health care providers, community leaders, service agencies, policymakers, funding sources, and target audiences
  • facilitate meetings and/or training for program participants
  • communicate consistently and effectively with the program director regarding any concerns or issues related to community outreach efforts
  • maintain BCFS HHS professional and ethical standards of conduct outlined in the employee handbook, demonstrating respect for agency staff, families, and community members, and complying with the required dress code at all times
