Job Requirements
- RN with current licensure in Kentucky and/or Indiana or ability to obtain
- minimum one year nursing experience
- knowledge of palliative care
- valid driver’s license
- ability to work 8 a.m.-8 p.m. on weekends and one weekday shift
- proficiency with technology and electronic medical records preferred
Job Qualifications
- Current RN licensure in Kentucky and/or Indiana or ability to obtain upon hire
- BSN preferred
- Certified Hospice and Palliative Nurse (CPHN) encouraged
- minimum one year nursing experience
- knowledge of palliative care required
- proficiency with electronic medical records preferred
- valid driver’s license as per policy
Job Duties
- Respond to on-call nursing service needs
- conduct patient visits including scheduled and unplanned visits
- perform admission visits for eligible patients
- facilitate facility to home admissions as needed
- maintain compliance with regulatory and accreditation standards
- demonstrate excellence in nursing practice
- support organizational nursing care goals
