Job Requirements
- Bachelor's degree in human or health services or related profession from an accredited university preferred or equivalent work experience
- Excellent customer service and conflict resolution skills
- Computer proficiency and ability to document accurate and timely notes
- Current driver's license and proof of insurance
- Ability to travel locally approximately up to 75% of the time
- Ability to lift and carry up to 15-20 pounds
- Ability to sit, stand, and walk for prolonged periods throughout the workday
- Ability to use standard office equipment
Job Qualifications

- Equivalent work experience accepted
- Excellent customer service skills
- Strong conflict resolution skills
- Proficiency with computer systems and documentation
- Experience in health care, elder care, social work, or related industry preferred
- Valid driver's license and current auto insurance
Job Duties
- Manage the day-to-day client relationship and delivery of quality care
- Identify and focus on a holistic approach to client care with a focus on quality of life
- Conduct in-person assessments with prospective clients, addressing their questions and concerns
- Build rapport with clients and their support system through interpersonal relationships and communication
- Ensure care team is up-to-date with relevant information and care plans are thoroughly documented and updated
- Collaborate and communicate regularly with internal and external customers
- Consult with clients regarding their needs, preferences, and objectives, evaluating potential solutions
- Act as subject matter expert for quality control and quality assurance
- Conduct client re-assessments including home safety, daily routines, and client preferences
- Proactively assess and refer clients to additional services or adjust caregiving staff
- Communicate with interdisciplinary team regarding care experience, caregiver matchmaking, billing accuracy, and care insurance benefit utilization
- Manage caregiver introductions and recommend additional caregiver training as needed
- Handle escalations and complaints utilizing judgment and discretion
- Review and adjust client rates based on care needs and service quality
- Ensure compliance with state regulations and company guidelines
- Recommend and implement policies and procedures
- Look for cross-referral opportunities to increase business
- Support on-call and after-hours as needed
- Perform additional duties as assigned
