Job Requirements
- associate degree in human services or related field
- three years of experience working with vulnerable individuals
- combination of education and relevant experience considered
- valid N.Y.S. driver’s license
- registered and insured vehicle
- N.Y.S. child abuse registry clearance
- mobility sufficient to drive to and participate in meetings at client’s homes and community facilities
- ability to work full-time at 35 hours a week

Job Duties
- Provide transportation for clients between hotels and shelters
- transport clients moving from Emergency Family Shelter to housing destinations
- transport clients to appointments and apartment viewings as needed
- conduct client interviews to assess personal and family challenges
- gather and evaluate medical, psychological, and social information
- provide counseling and support to develop individualized care plans
- refer clients to community resources and organizations
- maintain accurate documentation and reports for government agencies
- ensure communication with supervisors and stakeholders
- report safety issues in a timely manner
