Job Requirements

  • associate degree in human services or related field
  • three years of experience working with vulnerable individuals
  • combination of education and relevant experience considered
  • valid N.Y.S. driver’s license
  • registered and insured vehicle
  • N.Y.S. child abuse registry clearance
  • mobility sufficient to drive to and participate in meetings at client’s homes and community facilities
  • ability to work full-time at 35 hours a week

Job Duties

  • Provide transportation for clients between hotels and shelters
  • transport clients moving from Emergency Family Shelter to housing destinations
  • transport clients to appointments and apartment viewings as needed
  • conduct client interviews to assess personal and family challenges
  • gather and evaluate medical, psychological, and social information
  • provide counseling and support to develop individualized care plans
  • refer clients to community resources and organizations
  • maintain accurate documentation and reports for government agencies
  • ensure communication with supervisors and stakeholders
  • report safety issues in a timely manner
