Job Requirements
- high school graduation or G.E.D. equivalent and two years of direct services in mental health or community services
- or associates degree and one year of experience
- or bachelor’s degree in social services
- valid and current driver’s license
- personal vehicle insurance with your name listed as a driver
- willingness to use personal vehicle to drive clients to appointments and groups
- willingness to use personal vehicle to attend meetings

Job Duties
- assist individuals with mental illness and co-occurring disorders in living independently
- provide case management and support under direct supervision
- transport clients to appointments and community groups using personal vehicle
- attend meetings and collaborate with community partners
- maintain accurate and timely documentation
- monitor client progress and adjust plans accordingly
- facilitate communication between clients and service providers
