Job Requirements
- High School diploma or GED
- At least two years of progressively responsible work or volunteer experience in community-based health care or social work or at least one year experience as Case Manager I or equivalent
- Proficient skills using Microsoft Office applications like Word, Excel and Outlook
- Ability to work in or manage databases
- Access to reliable transportation with current license and insurance

Job Duties
- Outreach via telephone and in person at LifeLong community and residential sites to patients who meet case management eligibility criteria or prioritized by LifeLong
- Proactively meet and engage with patients to build relationships and assess strengths and needs using standard intake and screening tools
- Actively involve patients and caregivers in designing and delivering services including development of care plans aligned with patient goals
- Provide and facilitate referrals for internal and external resources and assist with completing applications and releases
- Maintain a patient caseload according to LifeLong standards and program requirements
- Utilize data registries and reports to manage caseload, maintain grant deliverables, and promote quality care
- Provide health education and training including harm reduction and disease risk mitigation strategies
- Assist patients with accessing public benefits and affordable housing
- Communicate respectfully and routinely with patients, care teams, external partners, and social supports
- Maintain knowledge of patients' medical and behavioral health plans and facilitate service utilization through accompaniment or transportation
- Participate in team meetings to coordinate care and support patient goals
- Advocate for patients to meet their needs and support self-advocacy
- Provide case management for complex acute or chronic health or behavioral conditions
- Conduct general housing case management including document readiness and assessments
- Assist with patient crisis intervention and de-escalation
- Provide and document billable services generating revenue for LifeLong
- Stay current on community resources and social supports
- Document patient contacts and services per policy
- Promote diversity, equity, inclusion and belonging
- Represent LifeLong positively and advocate for underserved populations
