Job Requirements
- Valid Texas Social Work license
- Bachelor’s or Master’s degree in Social Work from an accredited institution
- Must have own reliable transportation for travel and home visits
- Valid Class C Texas driver’s license or equivalent from another state
- Proof of appropriate auto insurance
- Ability to travel up to 60% within the region
- Ability to work nonexempt hours with possible alternate shifts for emergency response
- Must have immunizations as required for public health safety
- Maintain confidentiality and comply with HIPAA
- Pass background and due diligence checks as required
- Adherence to agency policies and procedures
- Participate in disaster response, recovery, or COOP activation as needed
Job Qualifications
- Undergraduate or graduate degree in Social Work from an accredited university
- Current license to practice Social Work in the State of Texas
- Experience in public health social work or case management
- Knowledge of Medicaid and related children’s health programs
- Skilled in conducting in-home assessments and developing service plans
- Ability to provide presentations and training
- Proficient in Microsoft Office and database software
- Strong communication and advocacy skills
- Ability to work independently and as part of a multidisciplinary team
Job Duties
- Conduct home visits for comprehensive family and child assessments
- Provide case management services for Children with Special Health Care Needs and related programs
- Conduct Personal Care Services and Community First Choice assessments and reassessments
- Serve as a liaison between clients and multiple service providers
- Provide training, consultation, and technical assistance to program contractors and providers
- Participate in community organization functions and collaborate with advocacy groups
- Complete administrative duties, reports, and supervise Social Work Interns
