Job Requirements
- High school diploma or GED
- three years related experience in direct service and case management field
- valid Connecticut driver's license
- regular and predictable attendance
- ability to attend two-day new hire orientation

- knowledge of case management principles and practices
- strong interpersonal and communication skills
- ability to work independently and as part of a team
Job Duties
- Provide individualized case management and life skills training
- assist clients in obtaining and maintaining permanent supportive housing
- deliver job development and job search support
- offer educational services and substance abuse education
- conduct client advocacy and crisis intervention
- work collaboratively with the Residential Behavioral Health Treatment care team
- support clients’ recovery from mental illness and substance use
