Job Requirements

  • Valid Texas driver’s license and proof of appropriate auto insurance
  • Ability to travel up to 60% for home visits and community meetings
  • Must provide own transportation
  • Pass background and employment eligibility checks as required
  • Maintain an acceptable driving record as per Texas Department of Public Safety standards
  • Flexible to work outside typical hours during public health emergencies
  • Willingness to participate in disaster response and continuity of operations planning
  • Maintain current immunizations and compliance with Department health standards

Job Qualifications

  • Undergraduate or graduate degree in Social Work from an accredited four-year university program
  • Current license to practice Social Work in the State of Texas
  • Experience in public health social work and case management services
  • Knowledge of Medicaid, Children with Special Health Care Needs, and related children’s health programs
  • Strong skills in family assessments, service planning, and case coordination
  • Proficiency in computer software including Microsoft Word, Excel, PowerPoint, Access, and Outlook
  • Effective communication and presentation skills
  • Ability to maintain confidentiality and work independently with initiative
  • Experience working in interdisciplinary teams and community collaboration

Job Duties

  • Conducts home visits for comprehensive assessment of home environment, psychosocial, economic, and special medical needs
  • Provides case management services under various children’s and health programs
  • Performs intake, family needs assessment, service planning, and follow-up services
  • Coordinates services with medical providers, Medicaid programs, and community resources
  • Conducts Personal Care Services and Community First Choice eligibility assessments and reassessments
  • Maintains case documentation and confidentiality in compliance with HIPAA
  • Provides training, consultation, and technical assistance to service providers and contractors
  • Participates in community organization functions and represents DSHS in interagency groups
  • Attends staff meetings, trainings, and professional development sessions
  • Completes administrative duties, monthly reports, and supervises social work interns
  • Participates in disaster response and recovery activities and continuity of operations plans
