Job Requirements
- High school diploma or equivalent
- Two years experience in social services, case management, or veteran services preferred
- Valid driver’s license and reliable transportation required
- Ability to work in diverse environments including field, office, and residential settings
- Strong communication and interpersonal skills

- Knowledge of housing programs, veterans benefits, and community resources
- Experience working with individuals experiencing homelessness, mental health, or substance use disorders preferred

Job Duties
- Establish rapport and build trust with veterans in various settings including shelters and outreach programs
- Support veterans in identifying housing, income, health, and stability goals through assessments
- Provide direct support and referrals for emergency shelter, food, hygiene, and transportation
- Assist veterans in accessing health care, mental health treatment, substance use recovery, and benefits enrollment
- Develop individualized case plans with measurable goals in health, employment, financial literacy, education, and community support
- Provide ongoing case management and coaching to help veterans progress toward goals
- Connect veterans to housing opportunities and coordinate with landlords and providers
- Help veterans transition into permanent housing and offer follow-up support
- Coordinate access to emergency financial assistance including hotel stays and utility payments
- Maintain timely and accurate client records and case management documentation
- Track progress and adjust case plans as necessary
- Collaborate with VCP staff, community partners, and service providers to ensure coordinated care
- Participate in team meetings, case conferences, and community events
- Build relationships with local employers, landlords, and support networks
- Represent VCP's mission professionally
- Provide guidance or mentorship to new staff
- Conduct regular safety walkthroughs of residential areas and report maintenance needs
- Ensure living spaces meet safety standards for veterans' well-being
