Job Requirements

  • Hold a current permanent Texas social work license (LBSW, LMSW, LMSW-AP, or LCSW)
  • Be bilingual in English and Spanish
  • Willingness and ability to travel up to 50 percent of the time
  • Possess a valid driver’s license and provide own transportation
  • Maintain current immunizations to meet health guidelines
  • Ability to work full time during day shifts
  • Availability to assist with disaster response and recovery efforts
  • Compliance with DSHS policies and standards
  • Good standing with the Social Work Licensing Board
  • Geographical preference to be based in Mercedes, Texas

Job Qualifications

  • Current permanent social work license in Texas (LBSW, LMSW, LMSW-AP, or LCSW)
  • Bilingual in English and Spanish
  • Knowledge of case management principles and practices
  • Experience with children’s health programs such as Medicaid and CSHCN
  • Skilled in conducting family assessments and developing service plans
  • Ability to use computer software including MS Word, Excel, PowerPoint, Access, and Outlook
  • Experience providing presentations and training
  • Strong conflict resolution, mediation, and problem-solving skills
  • Ability to work independently and as part of a team
  • Effective communication skills both written and verbal
  • Valid driver’s license with the ability to provide own transportation

Job Duties

  • Provide complex and specialized social work consultation and case management services to families with children who have health risks or special health care needs
  • Conduct home visits to assess client environments and needs
  • Complete intake, family needs assessments, service plans, and follow-up services
  • Assist families with eligibility determination processes for multiple programs
  • Provide information, referral services, and link families to community resources
  • Maintain documentation including reports, case summaries, and correspondence
  • Participate in community organization functions and interagency group collaborations
  • Provide training, presentations, and technical assistance to agencies and public
  • Conduct quality assurance activities such as peer chart reviews
  • Support regional and statewide disaster relief efforts and related public health emergencies
