Job Requirements

  • Master's degree or equivalent in social work, nursing, health, human services, occupational therapy, or physical therapy
  • valid licensure such as LICSW or RN recognized by the State of Vermont
  • case management accreditation preferred
  • minimum of three years experience in a clinical setting
  • at least two years of experience in a medical setting
  • two years experience in case management or a similar role
  • ability to work collaboratively in an interdisciplinary team
  • excellent communication and organizational skills

Job Qualifications

  • Master's in social work from an accredited school of social work
  • LICSW preferred
  • current RN licensure or compact licensure recognized by the State of Vermont
  • master's degree from an accredited institution in a health or human services field
  • bachelor’s of science or higher degree from an accredited occupational therapy program
  • bachelor’s of science or higher degree from an accredited physical therapy program
  • case management accreditation from a nationally recognized accrediting body preferred
  • minimum of three years experience in a clinical setting, including two years in a medical setting
  • two years case management experience or similar role preferred

Job Duties

  • Coordinate care and services of selected patient populations across the continuum of illness
  • promote effective utilization and monitoring of health care resources
  • collaborate with an interdisciplinary team to achieve optimal patient-centered, clinical, and resource outcomes
  • identify and intervene to address barriers to treatment, health outcomes, and discharge planning including biopsychosocial and other high-risk factors
  • serve as a vital link between the patient, healthcare team, payers, and the community
  • participate in onsite interviews and hiring events
  • support patients through case management processes
