Job Requirements

  • Must be 18 years of age or older
  • must have held a valid driver's license (not a permit or intermediate license) for at least 12 months
  • must have a reliable vehicle for client transportation
  • must be able to attend a two-week paid orientation Monday through Friday from 8 a.m. to 5 p.m.
  • valid car registration and vehicle insurance if providing transportation
  • successful completion of drug testing and background checks
  • compliance with all Department of Human Services regulations and registries
  • ability to exert up to 100 pounds of force occasionally
  • willingness to work non-traditional schedules based on client availability

Job Qualifications

  • High school diploma or equivalent preferred
  • six months of experience in human services preferred
  • working knowledge of computers
  • valid driver's license in good standing
  • ability to complete state-required trainings and certifications within mandated timeframes
  • good communication and interpersonal skills
  • ability to work both independently and as part of a team
  • commitment to maintaining confidentiality and professional boundaries

Job Duties

  • Provide direct support in accordance with individual service plans
  • assist with socialization, personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other daily living activities
  • accurately document progress and review service plans
  • manage individual's assets within budget and preferences
  • maintain professional relationships with individuals, families, and case managers
  • report instances of abuse, neglect, medical, or behavioral incidents
  • uphold confidentiality and respect individual rights
  • accompany individuals to medical appointments and communicate with medical providers
  • administer medications if assigned and maintain medication security
  • monitor health and respond to emergencies appropriately
  • prepare meals in accordance with dietary plans
  • attend orientation and ongoing training
  • work scheduled shifts and notify supervisor of absences
  • collaborate respectfully with team members
  • comply with safety policies and report hazards
  • safely transport individuals and use safety equipment properly
  • perform housekeeping and environmental maintenance
  • participate in safety drills and emergency procedures
  • perform other duties as required
