Job Requirements
- At minimum, high school diploma or GED required
- Experience in a fast-paced environment servicing many clients preferred
- Experience in healthcare, addiction medicine, community health, care coordination preferred
- Experience with building individualized care plans is a plus
- Knowledge of compliant documentation practices
- Lived addiction experience is a plus

Job Duties
- Connect with patients monthly to address needs and provide resources
- Connect and maintain relationships with community partners for patient resources
- Work closely with a dedicated team to provide exceptional patient care
- Participate in community awareness and engagement events to promote Crossroads and its mission to treat addiction
- Build and maintain individualized care plans
- Ensure compliant documentation practices
- Assist patients in accessing safe housing, food, and healthcare resources
