Job Requirements
- Bachelor's degree in social work, nursing, human services, public health, or related field
- Experience working with older adults or adults with disabilities
- Strong communication skills
- Strong assessment and relationship-building skills
- Ability to stay organized in a high-growth environment
- Reliable transportation for local travel
- Ability to handle multiple tasks efficiently
Job Qualifications
- Bachelor's degree in social work, nursing, human services, public health, or related field
- Experience working with older adults or adults with disabilities (1+ year preferred)
- Strong communication, assessment, and relationship-building skills
- Ability to move quickly and stay organized in a high-growth environment
- Knowledge of Wisconsin's Family Care program preferred
- Experience supporting family caregivers or working in home-based care preferred
- Familiarity with supportive home care standards and training requirements preferred
Job Duties
- Serve as the main point of contact for assigned families
- Support caregivers with coaching, training, and problem-solving
- Ensure caregivers meet all compliance, documentation, and training requirements
- Complete initial assessments to understand member needs, safety risks, and caregiver capacity
- Develop individualized support plans aligned to the member's MCO care plan
- Conduct scheduled check-ins (phone, virtual, or in-person) as required
- Identify changes, risks, or concerns and escalate when necessary
- Maintain accurate, timely case documentation
- Collaborate with Entyre nurses, operations teams, and MCO care teams as needed
- Report incidents or safety concerns promptly
- Demonstrate strong technical fluency in managing digital workflows
