Job Requirements

  • Bachelor's degree required, preferred in social work, human services, or related field
  • At least one year of experience working with elders or adults with disabilities
  • Valid driver's license and automobile insurance
  • Demonstrated experience in case management, care planning, and assessment in healthcare
  • Proficiency in common software and web-based applications including Microsoft Excel, Word, PowerPoint, and Teams
  • Ability to input data into universal portal

Job Qualifications

  • Bachelor's degree in social work, human services, or related field
  • At least one year of experience working with elders or adults with disabilities
  • Demonstrated experience in case management, care planning, and assessment in a healthcare setting
  • Proficiency in Microsoft Excel, Word, PowerPoint, and Teams
  • Ability to input data in universal portals
  • Master's degree in social work or related field preferred
  • Experience in Adult Foster Care or similar setting preferred
  • Certifications in gerontology or case management preferred
  • Social worker license from Massachusetts Board of Registration in Social Work preferred
  • Familiarity with using Hubspot preferred

Job Duties

  • Conduct comprehensive initial home assessments ensuring all necessary caregiver credentials and home modifications are complete
  • Monitor day-to-day LTSS through Mass Health including patient enrollment, credentialing, claims operations, prior authorization appeals, program integrity and reporting
  • Work with Mass Health members and families to resolve AFC provider network issues and respond to payment and policy queries
  • Gather information, perform psycho-social evaluations and facilitate referrals to healthcare and community services
  • Coordinate patient placement in compliance with established procedures
  • Collaborate with caregivers and care teams to define and implement individualized care plans
  • Perform regular home visits to supervise personal care services and review care plan progress
  • Maintain detailed case records documenting significant changes and observations
  • Provide ongoing training and support to caregivers
  • Collaborate with nurses, physicians and medical providers to ensure timely delivery of consumer services
  • Participate actively in discharge and transition planning
  • Conduct annual reviews to ensure continued suitability of qualified settings
  • Maintain continuous survey readiness and ensure compliance with all regulatory and accreditation standards
  • Perform other duties as assigned
