Job Requirements

  • Reside in southeast side of Lake County, IL or within 10 miles radius in northern Cook County, IL
  • At least one year experience in healthcare or case management
  • One year experience with Medicare and Medicaid recipients, long-term care services, Home and Community-Based Services (HCBS), or managed care organizations
  • Intermediate to advanced proficiency in Microsoft Office
  • Ability to use electronic medical records and various software applications
  • Exceptional communication and interpersonal skills
  • Ability to travel locally for face-to-face member and family meetings
  • Must pass Tuberculosis (TB) screening
  • Valid state driver's license and maintain personal vehicle insurance with state minimum coverage
  • Ability to work Monday to Friday, 8:00 AM to 5:00 PM CST with some flexibility
  • Reliable home internet meeting minimum speed requirements

Job Qualifications

  • Bachelor's degree in social sciences, social work, human services, or related field
  • Active unrestricted Licensed Practical Nurse (LPN) license in Illinois with one year experience in assessments and elderly care
  • Experience in healthcare and/or case management
  • Experience working with Medicare, Medicaid, long-term care, and managed care organizations
  • Intermediate to advanced computer skills including Microsoft Word, Excel, and Outlook
  • Ability to use electronic medical records and information software
  • Exceptional communication and interpersonal skills
  • Ability to travel regionally to meet members and care teams
  • Preferred: Active unrestricted Licensed Professional Counselor (LPC) in Illinois
  • Preferred: Active unrestricted Licensed Social Worker (LSW) in Illinois
  • Preferred: Bilingual or multilingual abilities in English/Spanish, Arabic, Vietnamese, Amharic, Urdu or other languages

Job Duties

  • Visit Medicaid members in their homes, Assisted Living Facilities (ALFs), and Long-Term Care Facilities
  • Ensure members receive services in the least restrictive setting to maintain optimal well-being
  • Identify and resolve barriers hindering effective care
  • Plan and implement interventions to meet care needs
  • Coordinate services and evaluate case management plans against personal goals
  • Monitor patient care progress through assessments and care planning
  • Guide members and families toward appropriate resources and facilitate communication with interdisciplinary teams and providers
