Job Requirements

  • Applicant must reside in Winnebago County, IL or within 10-mile radius of its border
  • one or more years of healthcare or case management experience
  • experience with Medicare, Medicaid recipients, long-term care services, Home and Community-Based Services (HCBS) or managed care organizations
  • intermediate to advanced computer skills
  • ability to use electronic medical records
  • exceptional communication and interpersonal skills
  • ability to travel locally for member and family visits
  • must pass Humana's Tuberculosis screening
  • valid state driver’s license and personal vehicle liability insurance complying with state minimum requirements
  • must have dedicated workspace to protect PHI and comply with HIPAA
  • reliable internet with minimum 25 Mbps download and 10 Mbps upload
  • flexible to attend occasional onsite meetings
  • able to work a full 40-hour schedule Monday to Friday
  • meet language assessment if applicable

Job Qualifications

  • Bachelor's degree in social sciences, social work, human services, or related field
  • active, unrestricted Licensed Practical Nurse (LPN) in Illinois with one year of experience in comprehensive assessments and elder care
  • one or more years of experience in healthcare or case management
  • experience working with Medicare and Medicaid recipients and managed care organizations
  • intermediate to advanced computer skills including Microsoft Word, Excel, Outlook
  • proficiency in electronic medical records
  • exceptional communication and interpersonal skills
  • ability to travel for face-to-face meetings
  • preferred active unrestricted Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW) in Illinois
  • bilingual or multilingual abilities in English/Spanish, Arabic, Vietnamese, Amharic, or Urdu

Job Duties

  • Visit Medicaid members in their homes, assisted living facilities, and long-term care settings
  • assess members to ensure receipt of services in the least restrictive setting
  • identify and resolve barriers to effective care
  • plan and implement care interventions
  • coordinate, monitor, and evaluate case management plans
  • continuously monitor patient care and progress towards outcomes
  • guide members and families towards appropriate care resources
