Job Requirements
- Registered or certified medical assistant
- minimum 1 year experience in a medical office or related field
- ability to interpret medical records, lab results, and appointment notes
- proficient computer skills
- excellent organizational and time management skills
- willingness to travel to local senior living communities

- 1+ years of experience working in a medical office, community engagement, or other related fields
- ability to interpret medical records, lab results, and appointment notes
- experience in collecting clinical information and surfacing changes to the appropriate care team member
- proficient computer skills including Microsoft Office applications
- excellent organizational and time management skills
- preferred experience with chronic care management or other care management programs
- willingness to travel to local senior living communities
Job Duties
- Conduct monthly in-person rounds for patients enrolled in Advanced Primary Care Management or Guiding an Improved Dementia Experience programs
- address patient and durable power of attorney inquiries via telephone
- respond to patient or caregiver messages received via the Curana Patient Portal
- assist patients with scheduling follow-up appointments with Curana Providers or specialists
- manage electronic health records and ensure accurate and up-to-date patient records
- coordinate documents needed for review or signature by a provider
- facilitate provider orders and escalate findings
- support provider scheduling
- assist with prior authorizations
- obtain patient records and diagnostic test results
- organize telehealth consultations
- collaborate with the Curana Interdisciplinary Care Team to ensure seamless communication within the health network
- perform other duties as assigned
