Job Requirements
- Registered or certified medical assistant
- Minimum 1 year of relevant experience in medical office or related fields
- Ability to interpret medical records, lab results, and appointment notes
- Experience collecting clinical information and communicating findings
- Willingness to travel locally to Senior Living Communities
- Proficient in computer usage including Microsoft Office
- Strong organizational and time management skills
Job Qualifications

- 1+ years of experience working in a medical office, community engagement, or other related fields
- Ability to interpret medical records, lab results, and appointment notes
- Demonstrated experience in collecting clinical information and surfacing changes to appropriate care team members
- Willingness to travel to local Senior Living Communities
- Proficient computer skills including Microsoft Office applications
- Excellent organizational and time management skills
Job Duties
- Conduct monthly in-person rounds for patients enrolled in Advanced Primary Care Management or the Guiding an Improved Dementia Experience programs
- Address patient and durable power of attorney inquiries via telephone
- Respond to patient or caregiver messages received via the Curana Patient Portal
- Assist patients with scheduling follow-up appointments with Curana Providers or specialists
- Manage electronic health records and ensure accurate and up-to-date patient records
- Coordinate documents needed for review or signature by a provider
- Facilitate provider orders and escalate findings
- Support provider scheduling
- Assist with prior authorizations
- Obtain patient records and diagnostic test results
- Organize telehealth consultations
- Collaborate with the Curana Interdisciplinary Care Team to ensure seamless communication within our health network
- Perform other duties as assigned
