Job Requirements

  • Registered or Certified Medical Assistant
  • one or more years of experience in a medical office, community engagement, or related fields
  • ability to interpret medical records, lab results, and appointment notes
  • experience collecting clinical information and reporting changes
  • willingness to travel to local Senior Living Communities
  • proficient computer skills
  • knowledge of Microsoft Office applications
  • excellent organizational and time management skills

Job Qualifications

  • experience in collecting clinical information and surfacing changes to appropriate care team members
  • willingness to travel to local Senior Living Communities
  • proficient computer skills and knowledge of Microsoft Office applications
  • excellent organizational and time management skills
  • experience working with chronic care management or other care management programs preferred

Job Duties

  • Conduct monthly in-person rounds for patients enrolled in Advanced Primary Care Management or the Guiding an Improved Dementia Experience programs
  • address patient and durable power of attorney inquiries via telephone
  • respond to patient or caregiver messages received via the Curana Patient Portal
  • assist patients with scheduling follow-up appointments with Curana Providers or specialists
  • manage electronic health records and ensure accurate and up-to-date patient records
  • coordinate documents needed for review or signature by a provider
  • facilitate provider orders and escalate findings
  • support provider scheduling
  • assist with prior authorizations
  • obtain patient records and diagnostic test results
  • organize telehealth consultations
  • collaborate with the Curana Interdisciplinary Care Team to ensure seamless communication within the health network
  • perform other duties as assigned
