Job Requirements
- Registered or Certified Medical Assistant
- one or more years of experience in a medical office, community engagement, or related fields
- ability to interpret medical records, lab results, and appointment notes
- experience collecting clinical information and reporting changes
- willingness to travel to local Senior Living Communities
- proficient computer skills
- knowledge of Microsoft Office applications
- excellent organizational and time management skills

- experience working with chronic care management or other care management programs preferred
Job Duties
- Conduct monthly in-person rounds for patients enrolled in Advanced Primary Care Management or the Guiding an Improved Dementia Experience programs
- address patient and durable power of attorney inquiries via telephone
- respond to patient or caregiver messages received via the Curana Patient Portal
- assist patients with scheduling follow-up appointments with Curana Providers or specialists
- manage electronic health records and ensure accurate and up-to-date patient records
- coordinate documents needed for review or signature by a provider
- facilitate provider orders and escalate findings
- support provider scheduling
- assist with prior authorizations
- obtain patient records and diagnostic test results
- organize telehealth consultations
- collaborate with the Curana Interdisciplinary Care Team to ensure seamless communication within the health network
- perform other duties as assigned
