Job Requirements
- Registered or certified medical assistant
- 1 plus years of relevant experience in a medical or community engagement setting
- ability to interpret medical records and laboratory results
- proficiency in computer applications including Microsoft Office
- willingness and ability to travel to local senior living communities
- excellent organizational and time management skills
- ability to communicate effectively with patients and healthcare providers
Job Qualifications
- Registered or certified medical assistant
- 1 plus years of experience working in a medical office, community engagement, or other related fields
- ability to interpret medical records, lab results, and appointment notes
- experience in collecting clinical information and surfacing changes to appropriate care team members
- willingness to travel to local senior living communities
- proficient computer skills and knowledge of Microsoft Office applications
- excellent organizational and time management skills
- experience with chronic care management or other care management programs preferred
Job Duties
- Conduct monthly in-person rounds for patients enrolled in advanced primary care management or dementia experience programs
- address patient and durable power of attorney inquiries via telephone
- respond to patient or caregiver messages received via the Curana Patient Portal
- assist patients with scheduling follow-up appointments with Curana providers or specialists
- manage electronic health records and ensure accurate and up-to-date patient records
- coordinate documents needed for review or signature by a provider
- facilitate provider orders and escalate findings
- support provider scheduling
- assist with prior authorizations
- obtain patient records and diagnostic test results
- organize telehealth consultations
- collaborate with the Curana interdisciplinary care team to ensure seamless communication
- perform other duties as assigned
