Job Requirements

  • Registered or certified medical assistant
  • 1 plus years of relevant experience in a medical or community engagement setting
  • ability to interpret medical records and laboratory results
  • proficiency in computer applications including Microsoft Office
  • willingness and ability to travel to local senior living communities
  • excellent organizational and time management skills
  • ability to communicate effectively with patients and healthcare providers

Job Qualifications

  • Registered or certified medical assistant
  • 1 plus years of experience working in a medical office, community engagement, or other related fields
  • ability to interpret medical records, lab results, and appointment notes
  • experience in collecting clinical information and surfacing changes to appropriate care team members
  • willingness to travel to local senior living communities
  • proficient computer skills and knowledge of Microsoft Office applications
  • excellent organizational and time management skills
  • experience with chronic care management or other care management programs preferred

Job Duties

  • Conduct monthly in-person rounds for patients enrolled in advanced primary care management or dementia experience programs
  • address patient and durable power of attorney inquiries via telephone
  • respond to patient or caregiver messages received via the Curana Patient Portal
  • assist patients with scheduling follow-up appointments with Curana providers or specialists
  • manage electronic health records and ensure accurate and up-to-date patient records
  • coordinate documents needed for review or signature by a provider
  • facilitate provider orders and escalate findings
  • support provider scheduling
  • assist with prior authorizations
  • obtain patient records and diagnostic test results
  • organize telehealth consultations
  • collaborate with the Curana interdisciplinary care team to ensure seamless communication
  • perform other duties as assigned
