Job Requirements

  • Valid driver's license and clean driving record
  • Reliable transportation for daily facility visits
  • Professional appearance and demeanor
  • Ability to pass background check
  • Comfortable using technology (smartphone, laptop, CRM, GPS)
  • Availability to attend some evening networking events

Job Qualifications

  • Experience in healthcare, senior living, or relationship-based sales
  • Familiarity with skilled nursing facilities, hospitals, or assisted living communities
  • Previous roles in community outreach, business development, or account management
  • Knowledge of Medicare, Medicaid, or VA benefits (helpful but not required)
  • Comfortable using technology such as CRM, email, GPS, and smartphone apps

Job Duties

  • Build and maintain relationships with discharge planners, case managers, social workers, and administrators at skilled nursing facilities, hospitals, and assisted living communities
  • Conduct 10-15 facility visits per week to introduce our services and support patient discharge needs
  • Attend healthcare networking events, community functions, and professional meetings
  • Coordinate lunch-and-learns, educational presentations, and facility tours
  • Partner with complementary healthcare providers (physical therapy, home health agencies, hospice) to create dual-discharge programs
  • Track all activities, referrals, and relationship development in our CRM system
  • Work closely with our care coordination team to ensure seamless client onboarding
