Job Requirements
- Valid driver's license and clean driving record
- Reliable transportation for daily facility visits
- Professional appearance and demeanor
- Ability to pass background check
- Comfortable using technology (smartphone, laptop, CRM, GPS)
- Availability to attend some evening networking events
Job Qualifications
- Experience in healthcare, senior living, or relationship-based sales
- Familiarity with skilled nursing facilities, hospitals, or assisted living communities
- Previous roles in community outreach, business development, or account management
- Knowledge of Medicare, Medicaid, or VA benefits (helpful but not required)
- Comfortable using technology such as CRM, email, GPS, and smartphone apps
Job Duties
- Build and maintain relationships with discharge planners, case managers, social workers, and administrators at skilled nursing facilities, hospitals, and assisted living communities
- Conduct 10-15 facility visits per week to introduce our services and support patient discharge needs
- Attend healthcare networking events, community functions, and professional meetings
- Coordinate lunch-and-learns, educational presentations, and facility tours
- Partner with complementary healthcare providers (physical therapy, home health agencies, hospice) to create dual-discharge programs
- Track all activities, referrals, and relationship development in our CRM system
- Work closely with our care coordination team to ensure seamless client onboarding
