Job Requirements

  • Bachelor's degree in social work, psychology, mental health counseling, psychiatric nursing, or pastoral counseling, or equivalent education/training per state regulations
  • experience in grief and loss counseling
  • current CPR certification
  • current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation

  • experience in grief and loss counseling
  • current CPR certification
  • current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
  • master's degree in social work, psychology, mental health counseling, psychiatric nursing, or pastoral counseling preferred

Job Duties

  • Establishes a plan of care addressing bereavement needs, detailing services and frequency up to 13 months post-patient death
  • demonstrates active listening and provides impactful suggestions during phone and home visits within LHC guidelines
  • leads and coordinates at least 12 grief support group sessions per year
  • maintains bereavement binder with information and attendance sheets from various events and presentations
  • provides individualized and compassionate care for each bereaved, documented in visit notes, bereavement risk assessment, care plan, and EGSS scores
