Job Requirements
- Current RN license in the state of employment or eligibility to obtain
- at least one year of nursing experience
- compassionate and dependable in urgent care situations
- ability to work evenings, nights, weekends, and holidays
- valid driver's license, automobile insurance, and reliable transportation
- current CPR certification

- compassionate and calm demeanor in urgent care situations
- excellent clinical judgment
- ability to provide care in patients' homes or facilities
- valid driver's license, automobile insurance, and reliable transportation
- current CPR certification
- experience in hospice, home health, oncology, or palliative care preferred
- familiarity with symptom management and after-hours care preferred
- strong documentation and electronic medical records skills preferred
Job Duties
- Provide direct patient care and support after normal business hours
- serve as the primary liaison between patients, families, physicians, and the hospice team during shifts
- respond promptly and compassionately to urgent calls and patient visits
- evaluate patient and family needs to guide clinical decisions and implement care plans
- collaborate with interdisciplinary teams including LPNs/LVNs and CNAs
- accurately document clinical visits and communications
- participate in quality assessment and performance improvement programs
