Job Requirements

  • Graduation from an accredited college or university with a bachelor's degree in social work or other human services field such as sociology, special education, rehabilitation, counseling or psychology
  • At least three years of full-time professional work experience in a healthcare facility, preferably in long-term care
  • Clinical and/or nursing experience preferred
  • Successful completion of feeding program training
  • Ability to communicate effectively in English
  • Ability to operate personal computer and relevant software
  • Physical ability to occasionally lift and/or move up to twenty-five pounds
  • Willingness to work in a patient-care environment

Job Qualifications

  • Bachelor's degree in social work or related human services field such as sociology, special education, rehabilitation, counseling, or psychology
  • At least three years of full-time professional experience in a healthcare facility, preferably in long-term care
  • Knowledge of Medicare, Medicaid, and private insurance policies
  • Supervisory and management experience
  • Strong communication and interpersonal skills
  • Proficiency in computer applications including word processing and spreadsheets
  • Ability to handle sensitive issues with patients and their families
  • Knowledge of sales and marketing strategies in healthcare

Job Duties

  • Record and log all referrals
  • Communicate with all referrals to facilitate successful admissions
  • Coordinate tours, clinical evaluations, payment coverage confirmations, and contract signings
  • Design and implement marketing plans to promote the facility
  • Chair the marketing committee and represent the nursing home at community events
  • Establish payer type for each admission and collaborate with the Business Office for billing
  • Track customer flow records for performance evaluation
  • Prepare monthly reports on admission activity and results
  • Communicate regularly with referral sources
  • Track bed availability including bed holds
  • Supervise Clinical Liaison and Secretary 2 - Social Services
  • Work closely with Nursing Home Administrator to ensure satisfactory admission processes
  • Provide customer service training as needed
  • Ensure continuity of care and quality service delivery throughout the admissions process
