Job Requirements
- Graduation from an accredited college or university with a bachelor's degree in social work or other human services field such as sociology, special education, rehabilitation, counseling or psychology
- At least three years of full-time professional work experience in a healthcare facility, preferably in long-term care
- Clinical and/or nursing experience preferred
- Successful completion of feeding program training
- Ability to communicate effectively in English
- Ability to operate personal computer and relevant software
- Physical ability to occasionally lift and/or move up to twenty-five pounds
- Willingness to work in a patient-care environment
Job Qualifications
- Bachelor's degree in social work or related human services field such as sociology, special education, rehabilitation, counseling, or psychology
- At least three years of full-time professional experience in a healthcare facility, preferably in long-term care
- Knowledge of Medicare, Medicaid, and private insurance policies
- Supervisory and management experience
- Strong communication and interpersonal skills
- Proficiency in computer applications including word processing and spreadsheets
- Ability to handle sensitive issues with patients and their families
- Knowledge of sales and marketing strategies in healthcare
Job Duties
- Record and log all referrals
- Communicate with all referrals to facilitate successful admissions
- Coordinate tours, clinical evaluations, payment coverage confirmations, and contract signings
- Design and implement marketing plans to promote the facility
- Chair the marketing committee and represent the nursing home at community events
- Establish payer type for each admission and collaborate with the Business Office for billing
- Track customer flow records for performance evaluation
- Prepare monthly reports on admission activity and results
- Communicate regularly with referral sources
- Track bed availability including bed holds
- Supervise Clinical Liaison and Secretary 2 - Social Services
- Work closely with Nursing Home Administrator to ensure satisfactory admission processes
- Provide customer service training as needed
- Ensure continuity of care and quality service delivery throughout the admissions process
