Job Duties
- Maintains and updates facility orientation material for all new admissions
- facilitates the completion of the patient/resident expectation questionnaire for all new admissions
- inspects and ensures all subacute rehabilitation rooms are properly cleaned, maintained, and beds are made ready for new admission
- welcomes new residents and their families into the facility
- introduces residents to the staff and provides a tour of the facility ensuring all questions are answered
- communicates daily with residents to ensure their needs are met
- identifies any resident concerns and ensures prompt resolution
- maintains a daily log with questions, concerns raised and follow up taken on steps to rectify any identified concerns
