Job Requirements

  • Bachelor's degree in social services, rehabilitation, ancillary health care profession or related field
  • Alternatively, relevant experience and certifications such as COTA, OT, PT accepted depending on contract
  • At least 3 years professional experience working with individuals with disabilities
  • Experience and knowledge of ADA regulations and sensitivity to disability and older adult issues
  • Experience gathering information related to ADA Paratransit eligibility
  • Experience with functional assessments of individuals with disabilities
  • Solid interviewing and observational skills
  • Ability to administer standardized evaluation tools following protocols
  • Clear communication skills
  • Intermediate proficiency in Microsoft Office Suite
  • Availability to work flexible hours
  • Adaptability to changing work environment
  • Ability to work independently
  • Ability to maintain confidentiality
  • Knowledge of public transportation and ADA paratransit regulations
  • Ability to work well with elderly and disabled individuals

Job Qualifications

  • Bachelor's degree in Social Services, Rehabilitation, Ancillary Health Care or related field
  • Relevant professional experience and specialized certifications such as COTA, OT, PT considered in lieu of degree depending on contract specifications
  • Minimum 3 years professional experience working with individuals with disabilities
  • Knowledge of Americans with Disabilities Act regulations and sensitivity to disability and older adult issues
  • Experience conducting information-gathering for ADA Paratransit eligibility
  • Experience in functional assessment of individuals with disabilities
  • Solid interviewing, assessment and observational skills
  • Ability to administer and score standardized evaluation tools accurately
  • Clear communication skills for reporting findings
  • Intermediate proficiency with Microsoft Office Suite
  • Excellent organizational and multitasking skills
  • Ability to work flexible hours and under limited supervision
  • Ability to adapt to a rapidly changing environment
  • Ability to make clear and factual observations
  • Preferred additional language skills
  • Ability to maintain confidentiality
  • Knowledge of public transportation systems and ADA complementary paratransit regulations
  • Ability to work well with elderly and individuals with disabilities

Job Duties

  • Determine final eligibility based on application, interview, functional assessment and supplemental information
  • Conduct in-person interviews to assess mobility capabilities and review supplemental information
  • Determine required functional assessments for applicants
  • Provide written eligibility determination documentation within ADA required time frame
  • Read and interpret ADA paratransit applications, intake and professional verification forms
  • Review interview results and follow up on assessments and professional verifications
  • Provide additional written documentation to Client upon request
  • Assist leadership in identifying process improvement opportunities
  • Conduct functional physical and/or cognitive assessments including physical functional assessments using indoor and outdoor courses, FACTS and MMSE
  • Apply assessment information to determine capability to independently ride fixed route transportation
  • Provide required forms to administrative personnel for data-entry and eligibility letter generation
  • Participate in staff meetings with leadership and/or Client
  • Participate in community outreach and education as needed
  • Participate in initial and ongoing training as required
  • Complete all necessary reports, paperwork and maintain applicant tracking logs
  • Input information into Client software and other appropriate software
  • Build rapport and communicate effectively with MTM and Client personnel
  • Participate in appeals process upon request
  • Maintain positive attitude interacting with individuals, support staff and caregivers
  • Work with management and Client to follow through on service issues, troubleshooting problems, concerns, complaints and training
  • Conform to MTM's HIPAA standards
