Job Requirements

  • Must comply with pre-employment I-9, physical, background check, references, application, orientation, and health screening

Job Qualifications

  • One year experience in a social or recreational program preferably in senior care
  • certified activity coordinator or equivalent professional certification or completion of 36 hours of specific training
  • strong organizational and time management skills
  • flexibility and creativity
  • bilingual in English and Spanish preferred
  • ability to collaborate with interdisciplinary teams
  • proficiency in electronic health records
  • strong communication and supervisory skills

Job Duties

  • Plan a comprehensive program tailored to needs, interests, and limitations of frail elderly persons to promote optimum health, independence, and self-care
  • evaluate participants' functional status and establish realistic goals and treatment plans using recreation therapy techniques
  • collaborate with social workers, physical therapists, occupational therapists, CNAs, and others to develop care plans
  • complete required progress notes and six-month reassessments
  • provide recreational therapy opportunities and instruction to family and others
  • supervise and coordinate Program Aids, Activity Aides, Community Service providers, and volunteers
  • develop and post a monthly calendar of events
  • assist with in-service training and community event planning
  • manage the Activity Program budget and purchasing requests
  • establish and coordinate an activity volunteer program with community organizations
  • assist participants with personal care
  • ensure compliance with CMS, DHCS, and other standards
  • meet administrative requirements including data capture and report submission
  • conduct performance evaluations and scheduling
  • maintain proficiency in EHR and documentation
  • conduct home, hospital, and SNF visits and assessments
  • participate in scheduled outings
  • stay current with regulations and standards
  • maintain safety standards and participate in safety programs
  • perform other related duties as assigned
