Job Requirements
- High school diploma or equivalency required
- Experience in geriatrics, recreation, or social administration preferred
- Experience with dementia care preferred
- CPR/First Aid certification preferred
- Ability to communicate effectively
- Physical capability to perform job duties including lifting up to 40 pounds and pushing up to 100 pounds
- Must complete annual influenza vaccination and TB skin test unless accommodation granted
- Ability to interact compassionately with elderly residents
- Proficiency in documentation and use of communication equipment
- Availability to work designated shifts and duties as assigned
Job Qualifications

Job Duties
- Assist Life Enhancement Mentor with organizing activities, completing monthly calendars, providing input and ideas for monthly newsletter, setting up and breaking down activities
- Promote and provide diverse opportunities for meaningful pursuits, recreation, exercise, social interaction, and personal routines
- Provide wellness programming including exercise and awareness of daily pleasures
- Place seasonal and special decorations to encourage resident awareness and interest
- Perform one-on-one interactions and in-room visits monthly
- Organize household activities ensuring compliance with federal and state regulations
- Participate in event root cause analysis
- Complete psychosocial and functional documentation
- Ensure resident rights and responsibilities are honored
- Communicate and collaborate with nursing, therapy, and dietary teams
- Maintain professional records and documentation
- Provide programming to maintain daily living functions
- Complete and facilitate individualized care plans
- Attend resident care plans and nutrition care planning meetings
- Communicate treatment plans and perform modalities
- Transport residents as needed
- Act as liaison across disciplines
- Perform additional duties as required
