myPlace Health

myPlace Health is an integrated care delivery organization dedicated to providing comprehensive, value-based healthcare services to older adults, particularly those with significant health and social needs.

Established in 2021 through a collaboration between SCAN Group and Commonwealth Care Alliance (CCA), myPlace Health operates under the Program of All-Inclusive Care for the Elderly (PACE) model.

Their goal is to enable seniors to live independently in their homes and communities for as long as safely possible.

Care Model and Services

The PACE model offers a holistic approach to senior care, integrating primary and specialty medical services, prescription drugs, vision, dental, and hearing services, physical and occupational therapy, transportation, and social activities.

These services are tailored to each participant's individual needs and typically provided at no cost to those eligible for Medi-Cal or both Medicare and Medi-Cal.

• Downtown Los Angeles – opened its first care center in early 2024, marking a significant milestone in serving vulnerable older adults in the region.

Recent Developments

In May 2025, SCAN Group acquired full ownership of myPlace Health, reinforcing its commitment to the PACE model and the well-being of older adults.
