• Ohio: Colony Healthcare Center in Tallmadge, Pines Healthcare Center in Canton, and Homestead Healthcare Center in Springfield. These centers provide skilled nursing, rehabilitation, and specialized programs tailored to resident needs.
• Maryland: Second-largest provider of skilled nursing care with 15 facilities and nearly 2,000 beds. Notable centers include Blue Point Healthcare Center and Northwest Healthcare Center in Baltimore, Clinton Healthcare Center in Clinton, and Kensington Healthcare Center in Kensington.
• Virginia: Facilities include Sleepy Hollow Healthcare Center in Annandale, Cedars Healthcare Center in Charlottesville, and Petersburg Healthcare Center in Petersburg. These offer rehabilitation, long-term care, and assisted living services.
• Indiana: Expanded footprint by acquiring 16 skilled nursing centers with over 1,000 beds in cities such as Indianapolis, Carmel, and Kokomo, providing rehabilitation and long-term care.
