Job Requirements
- Must hold current state RN and NP licensure
- Must hold a current Board Certification in a Nursing Specialty/Field
- Must possess a minimum of a Bachelors and Masters Degree in Nursing from accredited colleges or universities
- Minimum of 3 years palliative or hospice nursing experience required
- Demonstrated critical thinking skills required
- ACHPN certification is preferred but not required with willingness to complete within a year of employment
- Must possess knowledge of nursing and clinical nurse practitioner critical thinking skills application to the aged, chronically ill, and convalescent resident
- Strong leadership, interpersonal, and problem-solving skills preferred
- Must possess excellent written and oral communication skills
- Must possess traits of punctuality, organizational skills, and ability to function within a team
Job Qualifications

Job Duties
- Perform comprehensive health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities
- diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness
- provide ongoing care management that improves quality of life while helping residents remain healthier and more independent
- conduct regular facility rounds and develop strong relationships with residents, families, and care teams
- monitor changes in health status and proactively intervene to prevent complications and unnecessary hospitalizations
- educate residents and caregivers on disease management, preventative care, and wellness strategies
- partner closely with physicians, clinical staff, facility leadership, specialists, and family members to coordinate comprehensive care plans
- serve as a trusted clinical resource within the facility and contribute to interdisciplinary care discussions
- support initiatives focused on improving patient outcomes, enhancing quality measures, and reducing avoidable hospital readmissions
- maintain accurate and timely documentation within the electronic medical record (EMR)
- ensure compliance with applicable regulatory, clinical, and organizational standards
