The Housing Specialist role at Vaya Health is a vital position focused on supporting the organization's mission to help individuals live in the most integrated and appropriate housing settings. This remote position requires residency within North Carolina or within 40 miles of the state's border, with preference for applicants living in Buncombe, McDowell, or Caldwell counties. The Housing Specialist acts as a key liaison between individuals experiencing mental health, substance use, or developmental disabilities and the housing resources essential for their stability. Responsibilities include managing housing programs, facilitating access to housing subsidies, conducting community outreach, and engaging landlords and other stakeholders to maximize housing opportunities for the priority population, including those discharging from state psychiatric hospitals.

This role demands a comprehensive understanding of federal, state, and local housing regulations, as well as the ability to navigate complex inter-agency relationships. The Housing Specialist administers state and federally funded housing programs such as the Transitions to Community Living Voucher Program, ensures compliance with contractual obligations, supports housing waitlists, and provides training and expert guidance to internal and external partners. They also address social determinants of health by connecting individuals to valuable resources that extend beyond housing, including supportive services and legal aid. The work requires extensive collaboration across multiple disciplines and agencies to effectively coordinate housing solutions and discharge planning.

Job Duties

  • Administer state and federally funded housing programs and services
  • Manage housing initiatives, activities, programs, and funding as assigned
  • Support housing team in meeting reporting requirements
  • Maintain wait lists for housing programs
  • Determine member eligibility for housing services
  • Partner with local housing agencies to provide rental assistance and supportive services
  • Receive and process housing assistance requests
  • Complete annual grant applications within state and federal systems
  • Ensure integrity and compliance of housing programs
  • Maintain accurate records for housing budgets
  • Adhere to policies, procedures, and regulations
  • Support facility closure and discharge processes
  • Serve as point-of-contact for contractual and service issues
  • Administer Transitions to Community Living Voucher Program
  • Coordinate subsidy administration complying with DHHS and NCHFA requirements
  • Monitor compliance with HUD and housing codes
  • Collaborate with internal departments for process improvement
  • Implement leasing, tenancy, and subsidy processes
  • Collect and enter data into housing databases
  • Provide training and guidance on housing resources and regulations
  • Market subsidy programs to landlords
  • Mediate tenancy communication issues
  • Refer members to legal aid when needed
  • Maintain housing resources and vacancy reports
  • Develop strategies to address housing deficits
