The Case Manager/Housing Navigator role at Endeavors is pivotal in providing individualized case management and housing-focused services to shelter residents. Job Requirements
- Bachelor's degree in social work, human services, psychology, sociology or related field
- Minimum 2 years of related experience or training in homeless services, housing navigation, or case management preferred
- Ability to work flexible schedules including weekends, holidays, and overnight shifts
- Driver's license with a clear record
- Daily use of a vehicle without prior notice
- Must be available and willing to travel to various locations as business needs dictate
- Ability to maintain regular and acceptable attendance
- Strong interpersonal and communication skills
- Ability to maintain client confidentiality
- Familiarity with trauma-informed, client-centered, and harm reduction approaches
Job Qualifications
- Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or related field
- Minimum 2 years of related experience or training in homeless services, housing navigation, or case management preferred
- Experience with Housing First philosophy and strategies, tenant rights and responsibilities, and strength-based case management is a plus
- Proficiency in Spanish and English (written and spoken) preferred
- Strong interpersonal and communication skills
- Ability to work flexible schedules including weekends, holidays, and overnight shifts
- Ability to maintain client confidentiality
- Knowledge of community resources and supportive services
- Skilled in motivational interviewing and crisis intervention techniques
- Familiarity with HMIS or similar data management systems
Job Duties
- Develop and maintain partnerships with landlords, property owners, housing authorities, healthcare networks, outreach teams, and community service providers
- Research rental markets, maintain housing inventories and landlord databases, recruit new landlords through outreach and ongoing communication regarding program benefits
- Assess housing providers and available units to ensure safe, appropriate housing placements
- Assist clients with housing searches, housing applications, lease coordination, accommodation requests, move-in preparation, and securing documentation
- Educate clients on tenant rights, lease agreements, housing responsibilities, independent living practices, and tenancy skills
- Serve as liaison between tenants and property owners to address concerns and support positive tenancy outcomes
- Maintain a caseload of approximately 20-30 clients providing regular case management, engagement tracking, and documentation
- Conduct intake assessments, eligibility screenings, and comprehensive evaluations of client needs
- Develop individualized housing and service plans with goals promoting self-sufficiency and permanent housing
- Provide strength-based, trauma-informed, and client-centered case management using motivational interviewing, harm reduction, crisis intervention, de-escalation, and supportive counseling
- Assist clients with life skills development, budgeting, problem solving, scheduling, coping strategies, and coordination of care needs
- Facilitate referrals and linkages to employment, healthcare, mental health, substance abuse treatment, public benefits, and community-based services
- Participate in coordinated entry, case staffing meetings, trainings, and collaborative activities
- Maintain accurate client records, HMIS data entry, reports, travel expenses, discharge plans, and required documentation in compliance with regulations
- Demonstrate professionalism, patience, cultural sensitivity, compassion, and confidentiality
- Utilize trauma-informed, client-centered, and harm reduction approaches
- Perform other duties as assigned
- Demonstrate exceptional customer service prioritizing clients to support Endeavors’ mission
