Health Services

The Birches at Trillium Woods Health Center provides a range of health services, including memory care, skilled nursing, and rehabilitation. The memory care program, Heartfelt Connections – A Memory Care Program®, is a nationally recognized, person-centered, activity-focused program that emphasizes each resident's unique abilities.

The skilled nursing care is rated 5 stars by the Centers for Medicare & Medicaid Services (CMS), placing it among the top-rated nursing care options in Minnesota.
