Ethica Skilled Nursing

Ethica is a non-profit organization operating 52 skilled nursing centers and one assisted living facility across Georgia. These centers are accredited by the Joint Commission and are primarily located in rural and medically underserved regions. Ethica focuses on providing both long-term and short-term care. It emphasizes the emotional, social, and physical well-being of its patients.

NextStep Care

NextStep Care is a case management organization offering collaborative case management and advocacy for high-need, low-income individuals throughout Georgia. Serving all 159 counties, NextStep Care primarily enrolls members through Medicaid’s 1915(c) Elderly and Disabled Waiver Program. This includes programs like the Service Options Using Resources in Community Environments (SOURCE) and the Community Care Services Program (CCSP).

As of July 2023, NextStep Care serves over 5,600 waiver members, with SOURCE enrollment comprising 97% of their membership.

Affinis Hospice

Affinis Hospice is a Medicare and Medicaid-certified provider of hospice care, serving 146 counties from 11 regional offices across Georgia. Established in 2010, it provides comprehensive care for patients facing terminal illnesses, focusing on comfort and support for both patients and their families.

A significant portion of Affinis Hospice's patients reside in Ethica nursing centers, highlighting the integrated care approach between the two organizations.

CHSGa Home Health

CHSGa Home Health is a Medicare- and Medicaid-certified agency providing skilled nursing and therapy services in patients' homes. Established in 2012, it holds a Certificate of Need to serve 34 counties in Georgia.

Services include skilled nursing, physical therapy, occupational therapy, speech-language therapy, social work, and personal care by a home health aide. Offices are located in Albany, Athens, and Vidalia, serving a wide range of counties across the state.

Integra Rehabilitation

Integra provides comprehensive physical, occupational, and speech therapy services. It partners with various levels of care and healthcare providers to deliver high-quality rehabilitation services.

ElderCare Pharmacy

ElderCare Pharmacy offers pharmacy services to skilled long-term care nursing centers and assisted living centers throughout Georgia. Its goal is to enrich the lives of those they serve.

Community Ambulance

Community Ambulance provides emergency medical transport services, non-emergency medical transport, and other transportation services in three counties across Georgia.
