The Registered Nurse (RN) Home Health role in Corpus Christi, TX, is a critical position within the Optum Care Delivery team. This full-time position offers a competitive hourly wage ranging from $28.94 to $51.63, based on experience, education, certifications, and local labor markets. External candidates also qualify for a $5,000 sign-on bonus. As an RN in home health, you will perform nursing assessments, develop and implement plans of care, and evaluate patient outcomes in collaboration with physicians, patients, and caregivers following agency policies and protocols. Your responsibilities encompass providing skilled nursing services, including medication administration, wound care, therapeutic treatments, and patient education on disease processes, self-care, and prevention.
The role requires conducting ongoing patient assessments using the OASIS data set, managing care coordination with the clinical team, and ensuring documentation and communication of patient progress to all relevant stakeholders. You will participate in case conferences, care planning, and discharge processes, contributing to an integrated team approach. In addition to direct patient care, this position involves supervising licensed vocational nurses (LVNs) and home health aides, supporting their practice and documentation. The ideal candidate is knowledgeable in Microsoft Office applications and must have reliable transportation to travel to patient sites. This position values bilingual skills, especially in Spanish, and experience with OASIS documentation is preferred.

Job Requirements
- Graduate of an accredited school of nursing
- Current unrestricted Texas RN license or compact license
- BLS certification or ability to obtain within 30 days of employment
- 1+ years home health care or related environment
- Wound care experience
- Knowledgeable in Microsoft Office including Outlook, Word, and Excel
- Ability to read, analyze and interpret information in medical records and health plan documents
- Driver’s license and access to reliable transportation to travel to patient sites

Job Duties
- Performs nursing assessments and develops, implements and evaluates plans of care in conjunction with the physician, patient and caregivers per agency policy and procedure
- Evaluates the effectiveness of the plan of care, the patient’s response to care and outcomes of care and revises the plan of care as needed
- Re-evaluates patient nursing needs to include on-going assessments using the OASIS data set at appropriate time points
- Provides skilled nursing services that require substantial and specialized nursing care including medication administration, wound care, therapeutic treatments, and skilled teaching as ordered and indicated
- Participates in health promotion and disease prevention activities
- Initiates appropriate preventive and rehabilitative nursing procedures following the physician’s plan of care
- Coordinates services and scheduling with the Clinical Team Manager and Clinical Team Coordinator including recommendations for additional home health care services within 24 hours of Start of Care
- Assesses the patient’s condition during every home health care visit and communicates assessments to Clinical Team Manager
- Performs care management duties including notifying the physician of changes, obtaining physician orders, reassessing for recertification, supervising LVNs and home health aides, attending case conferences, and documenting discharge
