The Center Nurse role at Oak Street Health is a critical clinical care team member position adapting the principles of value-based care to improve patient health outcomes and reduce avoidable hospitalizations. Reporting to the Practice Manager, the Center Nurse undertakes comprehensive clinical assessments, chronic condition management, skilled nursing procedures, and patient education, all while collaborating closely with interdisciplinary teams. The Center Nurse supports both walk-in and telephonic triage to prioritize patient needs effectively and ensure appropriate care levels. Responsibilities include medication administration including vaccines, wound care, medication reviews, and onsite diagnostic testing like EKGs and point-of-care labs. The role demands a proactive approach to identifying patient care gaps, managing complex care plans, facilitating transitions from hospital to home, and utilizing standard protocols for medication management, prescription renewals, and prior authorizations.

This full-time position offers a typical pay range of $60,522.00 to $129,615.00 annually, with compensation influenced by experience, education, and geographic considerations. Additionally, the role is eligible for CVS Health bonuses, commissions, or short-term incentive programs.
