Job Duties
- Deliver exceptional care to the dedicated client in their home
- help execute comprehensive personalized weekly schedules and care plans
- provide skilled nursing services as prescripted under a client's plan of care that can be safely provided in the home, within your scope of practice
- counsel clients and family in meeting nursing and related needs
- meet all documentation requirements according to agency policy and procedure
- coordinate services by participating in case conferences, case management, team meetings, and staff meetings as indicated
- perform administrative functions such as documentation of completion of tasks and report changes in the client’s physical condition, mental capability or behavior
