Banner Home Care is currently seeking a skilled and compassionate Physical Therapist to join their Greeley team. This position plays a crucial role in delivering rehabilitative care directly within patients’ homes, providing a meaningful impact on their recovery and independence. The Physical Therapist will conduct comprehensive evaluations, develop customized treatment plans, administer hands-on therapy, and supervise Physical Therapy Assistants (PTAs). This role offers a high degree of autonomy, allowing therapists to manage their schedule efficiently while maintaining reasonable productivity expectations of 5-6 patient visits per day. Banner Home Care supports its therapists by supplying essential tools such as work tablets, phones, access to advanced therapeutic equipment including ultrasound, electrical stimulation, anodyne technology, theraband, and offers federal mileage reimbursement to cover travel expenses. Ideal candidates bring excellent communication skills, flexibility, adept documentation abilities, and a genuine passion for patient-centered care. Job Requirements
- Graduated from an APTA accredited school of Physical Therapy or equivalent
- possess a current Physical Therapy license within the state of practice
- hold BLS certification
- demonstrate clinical competency, experience, training, and education relevant to the job
- ability to use computer software applications for automated patient care
- familiarity with patient care equipment, evaluation tools, and therapeutic modalities
- able to operate copier, multi-line telephone, fax machine, computer, pager, and other office equipment
- must have Arizona Fingerprint Clearance Card if working for Banner Home Care/Hospice or Boswell Skilled Nursing Facility
- flexibility to work full-time weekend shifts and additional weekdays
- capable of meeting productivity standards
- effective communication and documentation skills required
Job Qualifications
