The role of Nurse Practitioner or Physician Assistant at Curana Health involves being a trusted clinical leader who provides relationship-based, proactive care directly to seniors in their communities. These healthcare professionals perform comprehensive health assessments, develop individualized treatment plans, and manage acute and chronic conditions with a strong focus on preventative care and long-term wellness. This position offers the opportunity to build meaningful patient relationships while helping to reduce unnecessary hospitalizations and improve the overall quality of life for older adults.

Job Requirements
- Education at master's degree level in nursing or physician assistant studies
- valid nurse practitioner or physician assistant license and current board certification
- experience in clinical care for seniors preferred
- ability to travel to assigned facilities

Job Qualifications
- Valid nurse practitioner or physician assistant license in state of operations and current board certification in your field

Job Duties
- Perform comprehensive health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities
- diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness
- provide ongoing care management that improves quality of life while helping residents remain healthier and more independent
- conduct regular facility rounds and develop strong relationships with residents, families, and care teams
- monitor changes in health status and proactively intervene to prevent complications and unnecessary hospitalizations
- educate residents and caregivers on disease management, preventative care, and wellness strategies
- partner closely with physicians, clinical staff, facility leadership, specialists, and family members to coordinate comprehensive care plans
- serve as a trusted clinical resource within the facility and contribute to interdisciplinary care discussions
- support initiatives focused on improving patient outcomes, enhancing quality measures, and reducing avoidable hospital readmissions
- maintain accurate and timely documentation within the electronic medical record (EMR)
- ensure compliance with applicable regulatory, clinical, and organizational standards
