Job Requirements
- Nurse Practitioner/APRN/Physician Assistant license
- Louisiana state license
- Eligible to work in the United States
- Experience in skilled nursing facility or post-acute care setting preferred
- Ability to work 24-40 hours per week
- Reliable commute or plan to relocate to Baker, Baton Rouge, or Zachary, Louisiana
- Strong communication and collaboration skills

Job Duties
- Provide specialized Physical Medicine and Rehabilitation (PM&R)/Physiatry services as a rehab consultant
- Represent CRC in your facility through strong clinical leadership and collaboration with the interdisciplinary care team
- Collaborate with PCP teams to manage conditions common in SNFs such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries
- Work alongside PT, OT, and SLP teams to improve patient function and quality of life
- Follow therapy patients to track their progress in therapy
- Assess and collaborate on management of barriers to therapy including joint or neuropathic pain, spasticity, neurogenic bowel, neurogenic bladder
- Attend multidisciplinary meetings and bridge the gap between therapy and internal medicine
- Engage in culture building opportunities and ongoing education including monthly didactic lectures and regional meetings
