Job Requirements

Nurse Practitioner/APRN/Physician Assistant license
Louisiana state license
Eligible to work in the United States
Experience in skilled nursing facility or post-acute care setting preferred
Ability to work 24-40 hours per week
Reliable commute or plan to relocate to Baker, Baton Rouge, or Zachary, Louisiana
Strong communication and collaboration skills

Job Duties

Provide specialized Physical Medicine and Rehabilitation (PM&R)/Physiatry services as a rehab consultant
Represent CRC in your facility through strong clinical leadership and collaboration with the interdisciplinary care team
Collaborate with PCP teams to manage conditions common in SNFs such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries
Work alongside PT, OT, and SLP teams to improve patient function and quality of life
Follow therapy patients to track their progress in therapy
Assess and collaborate on management of barriers to therapy including joint or neuropathic pain, spasticity, neurogenic bowel, neurogenic bladder
Attend multidisciplinary meetings and bridge the gap between therapy and internal medicine
Engage in culture building opportunities and ongoing education including monthly didactic lectures and regional meetings
