The Care Facilitator at ChenMed plays a crucial role as one of the first points of contact for patients entering the medical clinic. This position serves not only as a patient advocate but also as a dedicated provider of excellent customer service. The successful candidate will be responsible for both administrative and secretarial duties, including patient check-in and check-out processes, appointment scheduling, and ensuring that patient information is accurately documented and handled with confidentiality. The Care Facilitator is expected to proactively contribute to enhancing patient experience by maintaining a welcoming environment, supporting clinical staff through effective schedule management, and participating in internal communication and coordination to uphold ChenMed’s high standards of care.

In this key patient-facing role, the Care Facilitator also manages logistics related to transportation, medication and grocery deliveries, and coordinates rideshare services for patients. They are entrusted to verify insurance details, collect copayments, and assist with referrals and follow-up appointments as needed. Acting as an ambassador for ChenMed’s values, the Care Facilitator conducts outreach efforts such as "love calls" and sending personalized cards to patients, fostering warmth and engagement. This role demands meticulous attention to detail, strong communication skills, proficiency in using medical software and Microsoft Office Suite applications, and a genuine passion for serving others. Eligible candidates should ideally have a high school diploma or equivalent, with preference given to those holding medical assistant credentials and possessing experience in a medical clinic environment.

Job Requirements

  • High school diploma or equivalent
  • Minimum one year of work experience in a medical clinic or similar environment
  • Ability to communicate effectively and courteously with patients and staff
  • Proficiency in using computers and software including EMR systems
  • Strong customer service skills
  • Detail-oriented and able to maintain accuracy of data and reports
  • Knowledge of medical insurance processes
  • Ability to handle confidential patient information with discretion
  • Availability to work flexible hours including evenings, weekends, and holidays
  • Willingness to travel locally and regionally up to 10%
  • Preferred BLS certification
  • Preferred graduation from an accredited Medical Assistant program
  • Proficient in Microsoft Office Suite
  • Fluency in English
  • Positive attitude and strong listening skills

Job Qualifications

  • High school diploma or equivalent
  • Graduation from a nationally accredited Medical Assistant program preferred
  • Minimum one year of experience in a medical clinic or similar environment
  • BLS for Healthcare Providers preferred
  • Exceptional customer service skills
  • Working knowledge of medical insurance and EMR systems
  • Proficiency in Microsoft Office Suite including Excel, Word, and Outlook
  • Strong verbal and written communication skills
  • Ability to communicate professionally and courteously
  • Detail-oriented with strong listening skills

  • Knowledge of ChenMed medical products, services, standards, policies, and procedures
  • Ability and willingness to travel locally up to 10%

Job Duties

  • Interact appropriately with patients and record information during check-in, check-out, and scheduling
  • Conduct lobby checks to ensure cleanliness and organization
  • Serve as gatekeeper for PCP and Specialists schedules, ensuring Top 40 patients are scheduled every 13 days
  • Utilize computer systems to add patient information, print forms, schedule appointments, respond to emails, and generate new patient packets
  • Conduct "love" calls and mail birthday and greeting cards to patients
  • Answer telephones, take accurate messages, and direct communications appropriately
  • Clearly communicate information about clinical personnel to patients and others
  • Verify patient insurance, collect copays, and assist with scheduling follow-up and specialist referrals
  • Escort patients to exam rooms and ensure their comfort
  • Act as HEDIS champion by providing visibility of HEDIS needs and reporting hospitalizations
  • Present customer service surveys to patients and escalate issues for immediate resolution
  • Participate in daily and weekly huddles to update on patient details
  • Regulate and coordinate transportation, medication, and grocery deliveries, including rideshare scheduling
  • Maintain confidentiality of patient information and medical records
  • Review patient information for completeness and accuracy
  • Transmit correspondence via mail, email, or fax
  • Perform other duties as assigned.
