The Care Manager role at LIFEPlan COO NY is pivotal within the care management program, designed to connect members with vital services to support their health and overall quality of life. The Care Manager acts as a trusted partner and advocate, working directly with individuals with IDD, their families, and service providers to develop and monitor person-centered Life Plans. These plans are carefully crafted through comprehensive assessments to meet medical, behavioral, social, and community needs, ensuring that every member has access to healthcare, educational opportunities, employment resources, and community engagement. The Care Manager oversees coordination of services under several programs such as Health Home Care, HCBS Basic Plan Support, and State Paid Care Management services and may also support specific groups like Willowbrook Class Members.

This is not a remote-only position; it requires a dynamic professional comfortable with travel across the designated service area to meet members in their homes or community settings. The role combines office work, remote tasks, and field visits, ensuring that members receive consistent, quality care. Care Managers use electronic health records and telehealth technologies to maintain accurate documentation and facilitate effective communication. Key to success in this role is advocacy that promotes self-determination and community inclusion, resolved conflicts through team planning, and precise compliance with regulatory and organizational standards such as HIPAA and OPWDD requirements. Prospective candidates should be prepared to work flexible hours, including evenings and weekends when necessary, to meet member needs and deadlines. This position requires a valid driver's license and reliable vehicle to enable frequent travel between sites.

Job Requirements

  • Must be able to meet members in their homes or other community locations
  • travel to off-site locations required
  • must reside within New York State or a contiguous state within 100 miles of assigned office hub
  • valid driver’s license from New York or a contiguous state
  • dependable vehicle with proof of auto insurance
  • ability to work flexible hours including evenings and weekends
  • ability to secure private workspace for telehealth and confidential communications
  • familiarity with electronic health records and technology
  • strong organizational and documentation skills

Job Qualifications

  • Bachelor of Arts or Science degree with two years of relevant experience
  • or a license as a Registered Nurse with two years of relevant experience
  • or a master’s degree with one year of relevant experience
  • preferred degrees in health and human services, psychology, sociology or related fields
  • experience working with people with intellectual and developmental disabilities, case management, mental health or substance abuse fields preferred
  • excellent interpersonal, public speaking and written communication skills
  • ability to work autonomously
  • demonstrated professionalism and respect
  • strong commitment to serving people with IDD and their families

Job Duties

  • Deliver person-centered care management services in compliance with regulatory standards and internal policies
  • complete comprehensive assessments and reassessments
  • identify gaps in service provision and make appropriate referrals
  • develop, implement and monitor member Life Plans by leading interdisciplinary teams
  • resolve conflicts in person-centered planning processes
  • complete all required service documentation in a timely manner
  • maintain members’ eligibility and enroll them in necessary programs such as HCBS waiver
  • identify and access benefits and entitlements
  • ensure proper consent and maintain accurate electronic health records
  • coordinate access to healthcare and preventative services
  • manage transitional care and discharge planning
  • foster community inclusion through referral to resources
  • use health information technology including telehealth services
  • support member self-advocacy
  • attend meetings, trainings and supervision as required
  • travel to meet members and providers
  • adhere to incident reporting and safety guidelines
  • comply with all policies, standard operating procedures, and conduct standards
  • maintain confidentiality in accordance with HIPAA and privacy practices
  • perform other duties as assigned
  • possess valid driver’s license and reliable vehicle for frequent travel
